If you were a eucharistic minister (either at Mass or visiting the sick or housebound), reader, altar server, musician or welcomer before the pandemic, let us know if:
- you are now willing to continue in your volunteer role, or
- no longer wish to continue,

- which ministry you have been doing
- at which Mass and which church you might be willing to return to, either immediately or in the future.
